COVID Vaccine Instructions

Free COVID vaccines are being offered at Queen's Hospital Punchbowl everyday from 7:30am to 7:00pm.

​

Parking is available in the Miller garage and POB 3.

​

1. Download the 3 files. 

2. Read the EUA fact sheet.

3. Fill out the pre-screening form.

4. Look at the map to see where vaccine is given.

5. Make sure to bring your Hawaii Senior Care ID Badge.
